Applications for the 2019-2020 school year will be taken beginning April 1st. 

Please upload all required documents in order to ensure your application is complete and can be processed.





Public school based Pre-K and Head Start programs are state and federally funded and income based; therefore, there are limited openings available at each site.  Completing an application does not guarantee placement.  Students are accepted based on need,not on a first come, first served basis.  


The following documents must be uploaded to your online application in order for your application to be considered complete.  

1. Child's birth certificate

2. Up to date immunization/shot records

3. Proof of Residence (full page of electric bill with the service address visible)

4. Proof of Income for each working adult in the household

Proof of Income includes:

  • 2 current consecutive check stubs for the last two months
  • SNAP / Food stamp documentation with expiration date visible
  • Statement of Social Security benefits
  • Official letter from Employer stating place of employment, hourly rate of pay, and average number of hours worked per week.
  • Statement of Irregular Income for cash payments (official form will be provided)
  • DCFS fostert care placement agreement
  • TANF documentation
  • Child Support / Alimony 
  • Statement of No Income for unemployed members of the household (official form will be provided) or unemployment benefit letter
  • 2018 Income Tax return and W2s (W2s are not accepted without the corresponding tax return)



2019 - 2020 

Over Income?  
The link below is for those families that would like to be considered for seats remaining in our early childhood programs that are not filled by income qualifying families. Completing this form does not guarantee a seat for your child. 
Seats for over income income students are tuition based. 
Students are selected for these limited tuition based seats with consideration to academic and social screening results.  
Tuition based seats are only available if we do not fill all seats with students that meet the 200% federal poverty level income requirement.





The TOTS machine is located at Gonzales Primary School located at 521 North Burnside Ave. in Gonzales
